If you are in the background check application, and unable to proceed, the second disclosure is most likely preventing you from proceeding.
When you are on Step 3 signatures, the tabs at the very top of this form must be checked Green:
You are required to complete and sign the "Shieldhub_Disclosure_Authorization" and the second form "Background Investigation." The second form will look similar to this one:
When both forms have a green check mark, the application will allow you to continue. Please contact Support if you continue to experience challenges continuing.
